Dr. Lasco's dental office staff will fill out and mail insurance claim forms for free to any and all insurance companies. Please be prepared to provide accurate personal and insurance information so the staff can complete and mail the forms efficiently.
If your insurance company deals directly with our office, only the deductible needs to be paid at the first appointment of each year. If there is a remaining balance that the insurance company does not send directly to our office, you will be billed after we have received payment from the insurance company.
If we can not confirm with your dental insurance company that they will send payments directly to our office you will be required to pay in full at each appointment. A few dental insurances will only send payment directly to the patient. However, we will still fill out and mail all your claim forms for you at no charge.
Dr. Lasco is not a Participating Provider with any insurance carrier. Most insurance carriers allow you to receive treatment with a non-participating provider, but you must check with your individual dental insurance carrier to understand the details of your policy.
